    AIVER / LIABILITY:  I hereby consent to my child in participating in the program registered above. By agreeing to this form and permitting my son/daughter to participate in this program, I as the parent/guardian, and on behalf of the child, agree to release and hold without liability the FHA,EJFHA, EFHA, its agents, volunteers, Board of Directors and employees, from any and all claims for damages or bodily injuries arising out of participating in this field hockey program. I also understand and grant permission that my picture of participating in FHA, EJFHA, EFHA activities may appear on FHA/EJFHA or EFHA website and advertising. 

    CODE OF CONDUCT  (“COC”):   Edmonton Field Hockey, The Edmonton Junior Field Hockey Association (EJFHA) and The Edmonton Field Hockey Association (EFHA) abides by a strict Code of Conduct (“COC”) governing all coaches, players, officials and parents. The purpose of the Code is to ensure that all of those people associated with field hockey in Edmonton and Alberta treat each other with dignity and respect. The COC is a condition of membership for all players, parents, coaches and officials. All members of EFH, EJFHA and EFHA agree to abide by the COC and to be subjected to the consequences of violating the Code. It is understood that by enrolling into the program that players, parents and coaches are all bound by this COC and its validity is hereby a matter of record and is not dependent upon physical signature. All enforcement decisions made by the officials of (EFHA, EJFHA, EFHA, FHA )are final and are not subject to review. No refunds will be granted to those players dismissed from the program for violating the COC. 

    • PLAYERS
    • Understand that there WILL be changes to rosters throughout the season. These changes are the decision of the coach. All changes are made as needed due to scheduling, skill level assessments, absences and needs of teams at different times throughout the season.
    • Respect your coach, teammates, referees and opponents at all times.
    • Give 100% effort at each practice and game.
    • Know the rules of the game of field hockey and abide by them.
    • Support and encourage your teammates. Set a positive example.
    • Bring and wear the required equipment at all games and practices (Mouth guard, Shin pads & field hockey stick).
    • Display good sportsmanship at all times on and off the field.
    • Don't be afraid to make a mistake; that’s part of learning.
    • Learn from your coaches; practice hard; play smart. Have Fun!
    • Be modest in victory and gracious in defeat.
    • Keep in mind at all times that you are representing Club & Association, The City of Edmonton and the Province of Alberta.
    COACHES
    • A coach is a role model, who must act with the highest moral character possible.
    • Coaches must reinforce the self image of each player. A coach must be positive and constructive.
    • Respect the game of field hockey. Respect the officials.
    • Teach the rules of field hockey. Abide by those rules in practices and games.
    • Establish objectives for the team and players and inform them of team and individual goals.
    • Teach and practice good sportsmanship at all games and practices.
    • Be modest in victory and gracious in defeat.
    All
    1. Any conduct, including abusive language and/ or gestures, racially insensitive remarks, damage to property or belongings, or any other conduct that reflects poorly on yourself or (EFHA, EJFHA, EFH)is deemed to be unacceptable and will not be tolerated. This rule applies to all players, , and  guests at all times when in or around games or practices as a member of (EFHA, EJFHA, EFH).
    2. During a tournament or game when you are away, you represent the City of Edmonton, the province of Alberta and (EFHA, EJFHA, EFH))Any conduct such as destruction of property, consumption of drugs or alcohol, or any other activity will not be tolerated. This rule applies for the entire time one is away for a game or tournament.
    The ((EFHA, EJFHA, EFH)D irectors will deal with unacceptable conduct at its sole discretion, imposing warnings, suspension or expulsion, as it deems appropriate. ANY SUSPENSION OR EXPULSION BASED ON VIOLATIONS OF THE CODE OF CONDUCT WILL NOT BE ACCOMPANIED BY A REFUND.
