AIVER / LIABILITY:
I hereby consent to my child in participating in the program registered above. By agreeing to this form and permitting my son/daughter to participate in this program, I as the parent/guardian, and on behalf of the child, agree to release and hold without liability the FHA,EJFHA, its agents, volunteers, Board of Directors and employees, from any and all claims for damages or bodily injuries arising out of participating in this field hockey program. I understand that the coaches and managers are not responsible for my child/ward before or after the game or practice. Therefore if I or another adult of my choosing is not at the field before or after game or practice time it is understood that this means that my child/ward has permission from me to leave the field of play / facility on their own. I also understand and grant permission that my child’s picture of participating in FHA, EJFHA activities may appear on FHA/EJFHA website and advertising.
CODE OF CONDUCT (“COC”):
The Green & Gold Junior Field Hockey Club (GGJFHC)
sanctioned by the Edmonton Junior Field Hockey Association (EJFHA)
abides by a strict Code of Conduct (“COC”) governing all coaches, players, officials and parents. The purpose of the Code is to ensure that all of those people associated with field hockey in Edmonton and Alberta treat each other with dignity and respect. The COC is a condition of membership for all players, parents, coaches and officials. All members of (GGJFHC & EJFHA)agree to abide by the COC and to be subjected to the consequences of violating the Code. It is understood that by enrolling into the (GGJFHC & EJFHA)program that players, parents and coaches are all bound by this COC and its validity is hereby a matter of record and is not dependent upon physical signature. All enforcement decisions made by the officials of (GGJFHC & EJFHA)are final and are not subject to review. No refunds will be granted to those players dismissed from the program for violating the COC.
PARENTS
Respect the coach’s decisions in terms of team assignment. Please do not make
requests for placement prior to team publication nor make ANY requests or question the coaches’ changes to subsequent roster changes that will occur.
Understand that there WILL be changes to rosters throughout the season and these
changes are the final decision made by the coach. Parents must not intrude on this process.
Will teach and practice good sportsmanship at all games and practices. You are role models!
Know the rules of the game of field hockey.
Respect the game of field hockey. Respect the officials, the coaches, & the opponents.
Respect the individuals who are 100% volunteers.
Do not coach your child. Let players play and coaches coach, and support both positively.
Do not EVER criticize other players no matter what the circumstances.
Follow the "24 Hour Rule"; if you have a complaint resulting from a game (or practice) situation, speak with the coach on the following day.
If there is a serious problem, notify either Director of the Field Hockey Program.
Emphasize the FUN of the game and the team, putting forth effort at all times, not wins and losses.
Keep in mind at all times that you are representing the community & the city of Edmonton, and the province of Alberta.
Please understand that the directors/team managers of this program are 100%
parent volunteers. Without these volunteers there is no Field Hockey program
PLAYERS
Understand that there WILL be changes to rosters throughout the season. These changes are the decision of the coach. All changes are made as needed due to scheduling, skill level assessments, absences and needs of teams at different times throughout the season.
Respect your coach, teammates, referees and opponents at all times.
Give 100% effort at each practice and game.
Know the rules of the game of field hockey and abide by them.
Support and encourage your teammates. Set a positive example.
Bring and wear the required equipment at all games
and
practices (Mouth guard, Shin pads & field hockey stick).
Display good sportsmanship at all times on and off the field.
Don't be afraid to make a mistake; that’s part of learning.
Learn from your coaches; practice hard; play smart. Have Fun!
Be modest in victory and gracious in defeat.
Keep in mind at all times that you are representing Club & Association, The City of Edmonton and the Province of Alberta.
COACHES
A coach is a role model, who must act with the highest moral character possible.
Coaches must reinforce the self image of each player. A coach must be positive and constructive.
Respect the game of field hockey. Respect the officials.
Teach the rules of field hockey. Abide by those rules in practices and games.
Establish objectives for the team and players and inform them of team and individual goals.
Teach and practice good sportsmanship at all games and practices.
Be modest in victory and gracious in defeat.
All
Any conduct, including abusive language and/ or gestures, racially insensitive remarks, damage to property or belongings, or any other conduct that reflects poorly on yourself or (GGJFHC & EJFHA)is deemed to be unacceptable and will not be tolerated. This rule applies to all players, parents, siblings and guests at all times when in or around games or practices as a member of (GGJFHC & EJFHA).
During a tournament or game when you are away, you represent the City of Edmonton, the province of Alberta and (GGJFHC & EJFHA)Any conduct such as destruction of property, consumption of drugs or alcohol, or any other activity will not be tolerated. This rule applies for the entire time one is away for a game or tournament.
The (GGJFHC & EJFHA)Directors will deal with unacceptable conduct at its sole discretion, imposing warnings, suspension or expulsion, as it deems appropriate. ANY SUSPENSION OR EXPULSION BASED ON VIOLATIONS OF THE CODE OF CONDUCT WILL NOT BE ACCOMPANIED BY A REFUND.
